The safety and well-being of your pet(s) is of the highest importance to Beaver Brook Animal Hospital (BBAH).  Ensuring that your pet remains safe and well cared for is our first responsibility and as such we take it very seriously.

 

  1. I represent that I am the legal owner and at least 18 years of age of this/these pet(s)
  2. I release Beave Brook Animal Hospital, it's staff, owners, representatives, and agents from any and all liability which I or my pet may suffer including but not limited to injury, sickness, damage, or death resulting from participation in daycare, overnight boarding, or outdoor walks.
  3. I represent that my pet is in good health, is current on all required vaccinations;
    1. Canine Vaccines/Items Required
      1. DHPP, Rabies, (with Rabies Certificate), Bordetella, CIV (influenza)
      2. Current Town License
    2. Feline Vaccines Required;
      1. Rabies and Distemper
  4. I represent my pet is free of fleas, ticks and intestinal parasites and has not been ill with an known contagious viruses in the last 30 days.
  5. I represent that my pet is in good health, is current on all required vaccinations (DHPP, RABIES, Bordetella, CIV), is free of fleas, ticks and intestinal parasites and has not been ill with any known contagious viruses in the last 30 days.
  6. I understand while my pet is fully vaccinated that vaccines are not guaranteed and there is a small risk that my pet may contract a contagious disease or illness. I agree that should this occur I am responsible for my own pet’s care and medical attention.
  7. I understand that although pets are fully supervised that incidents of injuries may occur from playing with the other pets which include but not limited to bites, scrapes, scratches and sprains.
  8. I represent that my pet is sociable and has not harmed or shown threatening behaviors towards any person or any other pet.
  9. For canines, I understand BBAH reserves the right to remove my pet from the play area and place my pet in a separate holding area should my pet display unwanted behaviors.
  10. I understand that BBAH reserves the right to permanently remove a pet from its daycare or boarding facilities at any time.
  11. I allow BBAH staff to contact my veterinarian as deemed necessary should any injuries require previous medical history. I allow BBAH veterinarians and staff to treat my pet in the event of an injury or emergency.
  12. I agree to disclose any previous or current medical issues or concerns of my pet so that BBAH staff can make a determination of suitability for daycare.
  13. I release BBAH from any liability should my dog injure another pet or person and accept medical and legal responsibility for my dog’s actions
  14. I release BBAH from any liability should death of my pet occur for any reason while in boarding or daycare or grooming
  15. I agree that my pet may be recorded on camera and his/her images may be used for BBAH promotional material
  16. I agree to pay for any and all outstanding charges prior to or at time of pickup of my pet.
  17. I agree that BBAH will not be responsible or liable for lost, stolen, or damage to personal property belonging to me.
  18. If my pet becomes ill I understand that I will be notified pursuant to the contact information I have provided when my pet was checked in. If I am unable to be reached I agree to BBAH treatment and agree to promptly pay such expenses incurred in connection with such veterinary services.
  19. I agree that I have disclosed any allergies or special dietary needs to BBAH.

 

I certify that I have read, understood and agree with the policies of Beaver Brook Animal Hospital set forth on this page and that I agree with the conditions and statements of this agreement.
